Job Descriptions

Registered Nurse (RN)

The registered nurse plans, organizes, and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Qualifications

  • Registered nurse with current licensure to practice professional nursing in the state
  • Graduate of National League for Nursing accredited school of nursing
  • Maintains a current CPR certification
  • Current valid driver’s license
  • Minimum of two years experience, at least one of which is in the area of public health or home care nursing is preferred
Licensed Practical/Vocational Nurse

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Qualifications

  • Provides direct patient care as defined in the State’s Name Nurse Practice Act
  • Implements a plan of care initiated by the registered nurse
  • Provides accurate and timely documentation consistent with the plan of care
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
Certified Nursing Assistant

The certified nursing assistant is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The certified nursing assistant is responsible for observing patients, reporting these observations, and documenting observations and care performed.

Responsibilities of the certified nursing assistant include, but are not limited to the following:

  • Bath
  • Oral Hygiene
  • Shampoos
  • Changing Bed linen
  • Assisting patients with dressing and undressing
  • Skin Care to prevent breakdown
  • Assisting the patient with toileting activities
Provider

Home care providers render a wide range of home care services to seniors who want to age in place, securing their safety and well-being. These care professionals are well-trained and compassionate about attending to the needs of the client. They will render the following services:

  • Light Housekeeping
  • Cooking
  • Bathing and Toileting Assistance
  • Meal Preparation
  • Grocery Shopping
  • Running Errands
  • Medication Reminders
  • And More
Physical Therapist

The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

Qualifications

  • Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization
  • Licensed to practice as a physical therapist within the State of Texas
  • Two years of appropriate experience as a physical therapist
Physical Therapy Assistant
The physical therapy assistant provides physical therapy services under the direction of the physical therapist and according to the physician’s plan of care.
Occupational Therapist
The occupational therapist contracted through the Organization is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement.
